Registration forms must be submitted prior to attending a training session. Print and complete the registration, then submit it to the address or fax number at the end of this form.
Your personal information is being collected to register you for training delivered by CEAA and is collected pursuant to section 106(1)(c) of the Canadian Environmental Assessment Act, 2012 (CEAA 2012). Your personal information will be provided to CEAA trainers for course attendance/accommodations and to CEAA-Finance in order to process payment. Your personal opinions regarding training may be used to help improve the training that we deliver. Providing this information is voluntary however, missing information may result in you not being registered for available training. You have the right to access and correct any of your personal information, held by CEAA. Interdepartmental settlement codes are required to process payment of registration fees, if applicable. Please consult your Finance Officer for your departmental codes.
Cancellation of a registration, or a request to reschedule a registration for another course, will be accommodated at no charge if written notice is received by the Agency prior to the registration cancellation deadline (at least 15 working days prior to the course start date).
Registrants who fail to attend the course or who attempt to cancel or reschedule after the registration cancellation deadline will be invoiced for the full registration fee, where applicable. In exceptional circumstances, such as a serious illness (medical certificate required) or death in the family, the Agency may offer you a credit for attendance at a future course or refund. This will be determined on case-by-case basis.
Substitution of a participant is permissible with written notice of the change. The Agency cannot guarantee accommodation of special needs for changes made less than 15 business days prior to a course.
If registrants are unable to attend a course due to inclement weather, but the course has not been cancelled, they must notify the Training Coordinator on the day of the scheduled course. Registrants will be issued a credit toward another course. Cancellations due to inclement weather will not be accepted the day before or after the course.
The Agency reserves the right to cancel or reschedule courses where minimum participation is not reached or due to unforeseen circumstances. In such cases, full payment of registration fees will be returned to the registrants, where applicable.
